Where the cable connects to the magnet is so poorly designed and weak that the internal wires start breaking, and they eventually melt through the plastic coating. I am very careful with these things, and no matter what I do, they fry. If Apple isn't going to fix this problem, the very least they should do is allow other companies to make them--someone could undoubtedly design something better.
The MagSafe 2 magnetic connection is more shallow than the MagSafe was, which could reduce the risk of poor connection caused by a bend on the connector. You could use an adapter to convert a MagSafe to a MagSafe 2 connection, but I don't think it will cause it to charge more efficiently.

Bought this adapter new in April 2013 as a replacement for the family macbook 3.1 adapter (macbook is 64 months, 1300 + cycles, 80% battery capacity). This new adapter's pin is similar to the adapter on the Macbook Unibody (version 6.1). The old original T-shaped adapter's pins got permanently stuck in its tiny casing and was useless even though the main power unit was functional.
